CLINICAL EXPERIENCE USING THE COMBICAST® SEMI-RIGID BANDAGE

Autoři předkládají srovnání fixace zhotovené z materiálu CombiCast^® s běžně užívanou koncepcí znehybňování sádrovou fixací a její vliv na délku následné rehabilitace (RHB). Hodnocená skupina obsahuje 32 pacientů (19 mužů, 13 žen) ve věku 14-49 let s diagnostikovanou a operačně řešenou totální rupturou lig. fibulotalare anterius (FTA). Všem pacientům byla naložena ihned po operaci sádrová dlaha. První soubor obsahuje 15 pacientů, u nichž byla 10. den po operaci vyměněna sádrová fixace za semirigidní fixaci CombiCast^® na dobu 4 týdnů (průměrná doba celkové fixace 39 dnů). V druhém souboru 17 pacientů byla sádrová fixace naložena po celou dobu imobilizace, tj. 6 týdnů (průměrná doba ﬁxace 42 dnů). Celková doba rehabilitace byla u prvního souboru 31,2 dne (26-37 dnů) na jednoho pacienta. V druhém souboru byla celková doba RHB 40,6 dne (28-54 dnů) na jednoho pacienta. Lze konstatovat, že použitím funkčního znehybnění semirigidní fixací CombiCast^® se statisticky významně zkracuje doba RHB (p < 0,05) a tím i celková doba léčby ruptury FTA.

A comparison of the CombiCast^® Semi-rigid bandage with traditional immobilization by plaster of Paris and its inﬂuence on rehabilitation time is presented by the authors. 32 patients (19 males, 13 females) aged from 14 to 49 years and suffering from recent lateral ankle ligament injuries (grade III) — rupture of the anterior talofibular ligament (FTA) are presented. All patients were treated by the operative technique and their ankles were immobilized immediately by a plaster of Paris splint. The plaster of Paris splint was exchanged for a CombiCast^® semi-rigid bandage in the ﬁrst group of 15 patients on the 10th day following the operation. The CombiCast^® application lasted 4 weeks (the mean total time of immobilization was 39 days). The plaster of Paris was applied for the whole time of immobilization (6 weeks) (the mean total time of immobilization was 42 days). The total rehabilitation time (TRT) in the first group was 26-37 days (mean 31.2). The second group’s TRT was 28-54 days (mean 40.6). It’s concluded that the use of functional treatment with a CombiCast^® semi-rigid bandage reduces TRT (p<0. 05) and the total time of FTA rupture treatment as well.
