Malignant Melanoma

Autoři předkládají přehledný článek o maligním melanomu, strategii jeho léčby. Maligní melanom je zhoubný nádor vycházející z melanocytů. Jeho incidence v posledních 30 letech má trvale vzestupný trend. Příčina vzniku melanomu není přesně známa, ale mezi hlavní etiologické faktory se počítá sluneční záření.
Dělení na klinická stadia onemocnění je založeno na histologickém zhodnocení podle Breslowa (1970) a Clarka (1969). Podle Breslowovy klasifikace byla stanovena kritéria mortality. Ta je založena na hodnocení hloubky invaze měřené v milimetrech. Pro rozpoznání maligního melanomu byl vytvořen pomocný akronym ABCDE (Asymetry, Border, Color, Diameter, Enlargement).
Základ terapie tvoří chirurgická excize, dále podle klinického stadia následuje imunoterapie, popřípadě chemoterapie, chemoimunoterapie a radioterapie. Prognostické faktory zahrnují mnoho parametrů, z nichž nejvýznamnější jsou tloušťka nádoru, stav sentinelové uzliny, ulcerace nádoru a hloubka invaze. Prevence je založena na ochraně proti slunečnímu záření a používání ochranných krému s vysokým ochranným faktorem. Dále je to včasné chirurgické odstranění měnících se névů.

The authors present a well arranged article concerning malignant melanoma and the strategy of its treatment. Malignant melanoma is a malignant tumour which is created from melanocytes. Its incidence has a permanently increasing trend in the last 30 years. The cause of melanoma origin is not exactly known. But the sun is considered one of the main aetiological factors.The clinical stages of this disease are classified based on histological evaluation according to Breslow (1970) and Clark (1969). Mortality criteria were determined according to Breslow's classification. This classification is based on evaluating the invasion depth which is measured in millimetres. An auxiliary ABCDE (Asymetry, Border, Color, Diameter, Enlargement) acronym was created for a good recognition of malignant melanoma.The most important part of the therapy is a surgical excision. Then depending on the clinical stage immunotherapy or chemotherapy, chemoimmunotherapy and radiotherapy follow. The prognostic factors include many parameters out of which tumour thickness, sentinel lymph node condition, tumour ulceration and invasion depth are the most important. Prevention is based on the protection against sunshine and on the use of creams with a high protective factor. Changing naevi should also be removed surgically as soon as possible.
