Primary Torsion of the Omentum Imitating Acute Appendicitis - A Case Report

Torze omenta je vzácnou příčinou náhlé příhody břišní projevující se ve většině případů stejnými příznaky jako akutní apendicitida. Správná diagnóza bývá často stanovena až při operaci. Na možnost torze omenta může chirurga upozornit přítomnost serosanguinolentního výpotku v břišní dutině při normálním nálezu na apendixu. Léčbou je resekce postižené části omenta. Autoři předkládají kazuistiku muže operovaného pod klinickým obrazem akutní apendicitidy, kde příčinou stavu byla primární torze omenta s mnohonásobnou rotací 900 stupňů proti směru hodinových ručiček.

Torsion of the omentum is a rare cause of acute abdomen. In most cases, it is presented with the same symptoms like acute appendicitis. Usually, the correct diagnosis is not made until the operation. The presence of serosanguineous fluid in the peritoneal cavity and normal appendix can alert the surgeon to the possibility of omental torsion. The treatment consists in excision of the affected portion of the omentum. The authors present a case report of a young man operated on for signs of acute appendicitis where primary omental torsion with a multiple rotation 900 degrees in anticlockwise direction was the cause of acute abdomen.
